About Course

The 4C High Risk Obstetrics Course was run live by RIRS Guwahati, in association with the Association of Obstetric Anaesthesiologists (AOA), India, across eight evening sessions from November 2025 to January 2026. Every session is here in full, edited and chaptered.

Twenty-two talks take you from the physiological changes that make a pregnant patient different, through the emergencies that turn a routine delivery critical – valve lesions and cardiomyopathy, hypertensive crisis and eclampsia, VTE and massive haemorrhage, fluids and uterotonics, sepsis and tropical infection, imaging dilemmas, fetal monitoring, HELLP and AFLP – and end at maternal cardiac arrest and resuscitative hysterotomy.

Every lesson ends with multiple-choice questions written by the faculty, so you can check what stuck. Watch at your own pace, on any device. Access does not expire.

Course Content

Session 1 – Anatomy and physiological changes
Dr Saswati Sinha on the cardiovascular, respiratory, haematological and gastrointestinal changes of pregnancy and where they bite clinically; Dr Anju Grewal on the obstetric airway, AIDAA 2025 and a rehearsed plan for the failed intubation.

Session 2 – Cardiovascular emergencies in pregnancy
Dr Sai Rama Devi on stenotic and regurgitant lesions through a case; Dr Manokanth Madapu on peripartum and hypertrophic cardiomyopathy; Dr Anitha Prashanth on the spectrum of cardiac emergencies, from arrhythmia to aortic dissection.

Session 3 – Hypertensive module
Dr Neha Singh on definitions, prediction and the current guidelines; Dr GL Ravindra on hypertensive crisis and acute pulmonary oedema, drug by drug; Dr Saswati Sinha on peripartum seizures and when eclampsia is not the diagnosis.

Session 4 – Embolism and haemorrhage
Dr Anitha Prashanth on VTE in pregnancy and the YEARS algorithm; Dr Neha Singh on anticoagulants and bridging around delivery; Dr Nidhi Bhatia on haemorrhagic shock and haemostatic resuscitation.

Session 5 – Fluids and uterotonics
Dr Ankur Khandelwal on goal-directed fluid and blood resuscitation in a placenta accreta case; Dr Farzana Zahir on oxytocin, carbetocin, misoprostol and ergometrine, occlusion balloons and vascular clamps.

Session 6 – Infection and sepsis
Dr Sharmili Sinha on oliguria and acute kidney injury in the critically ill parturient; Dr Rakesh Periwal on dengue, malaria, scrub typhus and the tropical infections you will actually meet.

Session 7 – Imaging, fetal monitoring and septic shock
Dr Sai Rama Devi on CTG interpretation and fetal monitoring in the ICU; Dr Nilesh Naphade on imaging dilemmas and radiation in the critically ill pregnant patient; Dr Kajal Jain on maternal septic shock.

Session 8 – HELLP/AFLP, delivery dilemmas and maternal collapse
Dr Manokanth Madapu on HELLP and acute fatty liver of pregnancy; Dr Kalpana Choudhury on decision-to-delivery dilemmas in the ICU; Dr Nidhi Bhatia on maternal cardiac arrest, CPR modifications and resuscitative hysterotomy.
